The 60-Second Habit That Changes Appointments (and Your Medical Record)

Most patients aren’t failing the system. The system is failing to capture what’s actually happening.
Here’s the simplest tool I’ve ever found for getting clarity fast—and getting it into the record.
The “2–1–1–1” Method (60 seconds)
Before your next appointment, write:
Top 2 symptoms (what changed + when it started)
Top 1 impact (what it’s interrupting in your day)
Top 1 question (what you need answered today)
Top 1 next step (test, referral, note, med change, accommodation, etc.)
That’s it.
It works because it forces the visit to orbit around what matters most—and makes it harder for the important parts to get lost.
